CLARIFICATION TEXT ON THE PROTECTION OF PERSONAL DATA

Özkara Sağlık Hizmetleri ve Ticaret Anonim Şirketi (Düzey Göz) has the title of data controller pursuant to Article 3 Personal Data Protection . Your personal data may be processed in accordance with the Private Hospitals Regulation and the Regulations of the Ministry of Health and other legislative bodies. The framework is described below.

Which personal data is processed?

In order to provide healthcare services to our patients, we collect various personal data in accordance with the principles and conditions within the scope of KVKK and other relevant legislation. In order to provide healthcare services to our patients, the following data are collected by our medical center from our patients, other health institutions and organizations or patients’ relatives in order to carry out medical diagnosis, examination, treatment and care services:

• Your name and last name
• National Identification information and passport number
• Place of birth and date of birth
• Your gender and marital status
• Your address
• Your phone number
• Your e-mail addresses
• Your patient protocol number from Özkara Sağlık Hizmetleri ve Ticaret Anonim Şirketi and other identification data to identify you,
• Your data specified in the consent form that you may give in the event of a medical intervention,
• Private health insurance or Social Security Institution data for the purpose of financing and planning health services,
• Your general and special categories of personal data, especially personal health data obtained during the execution of all medical diagnosis, examination, treatment and care services,
• Closed circuit camera system images and recordings during your visit to our hospitals,
• Your survey, suggestion, thank you and complaint data,
• Other data you share when you contact us via e-mail, website contact form or other channels.

Your personal data is processed for the purposes listed below:

• To be able to purchase the products and services offered to you by our medical center and to benefit from such products and services,
• Identification and verification,
• Protection of public health, medical diagnosis and treatment services,
• Fulfillment of legal obligations in the relevant legal regulations,
• Planning and managing the internal functioning of our medical center and daily operations,
• Measuring, increasing and researching patient satisfaction within the framework of patient rights and patient experience activities by the management of our medical center,
• Supply of special medicines and/or medical supplies and/or devices to patients,
• In case of an appointment, informing patients about the appointment, providing information and/or reminding them of the appointment,
• Fulfillment of risk management and quality improvement activities,

• Sharing the information requested by the patient services, financial affairs and marketing departments with private insurance companies within the scope of financing your health services, covering your examination, diagnosis and treatment expenses, eligibility inquiry and financing of health services,
• Analyzing the use of health services and storing your health data in order to develop and improve the health services offered, responding to your questions or complaints regarding our services,
• Execution of patient relationship management processes,
• Realization of finance, accounting and invoice processes,
• Follow-up of request/complaint processes,
• Ensuring physical space security,
• To be able to carry out activities such as organizations, events, etc,
• Execution of contract processes,
• Informing about developments in the health institution, campaigns, new treatment methods, service areas and services to be offered, opening information and similar issues,
• Informing authorized persons and institutions,
• Creation and follow-up of visitor records,

Apart from the above-mentioned situations, your personal data may be processed for the purposes of informing you about developments in the health institution, new treatment methods, service areas and services to be offered, opening information and similar issues, provided that your consent is obtained.

Special sensitivity is shown by our medical center in the processing of sensitive personal data, which is believed to be more critical for data owners in various respects. In this context, such data are not processed without the explicit consent of the data owners, provided that adequate measures determined by the Board are taken.

To Whom And For What Purpose Personal Data Can Be Transferred

Your personal data, within the scope of the Law and other legislation and for the above-mentioned purposes by Özkara Sağlık Hizmetleri ve Ticaret Anonim Şirketi, private insurance companies, Ministry of Health and related units, Social Security Institution, General Directorate of Security and other law enforcement agencies, General Directorate of Population, Turkey Pharmacists Association, opticians and all kinds of judicial authorities, central and other third parties, Your authorized representatives, lawyers, third parties from whom we receive consultancy including accounting, tax and financial consultants, regulatory and supervisory authorities, our business partners and other third parties with whom we cooperate to develop or carry out health services for the above-mentioned purposes, including official authorities, or may be transferred abroad within this scope.

Method and Legal Reasons for Collecting Personal DataYour Personal Data Is Processed For The Purposes Listed Below:

Your personal data are collected and processed in all kinds of verbal, written, visual or electronic media for the above-mentioned purposes and in order to carry out all kinds of work within the scope of our Medical Center’s field of activity within the legal framework and to ensure that Özkara Sağlık Hizmetleri ve Ticaret Anonim Şirketi can fully and duly fulfill its contractual and legal obligations. The legal reason for collecting your personal data;

In addition, as stated in Article 6, paragraph 3 of the Law, health and personal data can only be processed for the protection of public health, preventive medicine, medical diagnosis, treatment services, health services, by persons under the obligation of confidentiality or authorized institutions and organizations without seeking the explicit consent of the person concerned.

Other Rights Of The Data Subject Whose Personal Data Is Processed Pursuant To Article 11 Of The LPPD.

• In case of damage due to unlawful processing of personal data, to demand compensation for the damage.

EXPLICIT CONSENT TEXT ON THE PROCESSING OF PERSONAL DATA

I have read and understood the entire “DISCLOSURE TEXT” consisting of 7 articles prepared by the MEDICAL CENTER within the scope of the Law No. 6698 on the Protection of Personal Data (“KVKK”), and I have been informed about which personal data is processed and how, and that I can withdraw my explicit consent at any time without any limitation.

In this context, for the processing activities specified in the content of the Clarification Text that my explicit consent must be obtained; I hereby accept and declare that I freely consent to the processing and transfer of my personal data to the specified persons, provided that my personal data is limited to the specified purposes, for the maximum period required and presumably for the legal statute of limitations, and that I have been informed by the MEDICAL CENTER in a manner that does not leave any room for doubt about these processing activities.

I hereby accept and declare that I give explicit consent to the transfer of my personal data to data processing service providers such as the call center included in the Clarification Text by the MEDICAL CENTER.
